Welcome to the provider portal for Simple Solutions. I am Kathy Garrett, and I serve as a dedicated extension of your clinical team, providing the intensive behavioral education necessary to manage complex metabolic conditions.
What are your professional credentials?
I hold a Bachelor of Science in Nutrition Education, which provides the academic foundation for my clinical approach. Additionally, I am:
- CHES® (Certified Health Education Specialist): Nationally board-certified in the design and delivery of health interventions.
- NASM-CPT (Certified Personal Trainer): Qualified to implement safe, evidence-based physical activity protocols.
- NPI Registered: I am a credentialed provider with a National Provider Identifier.
What specific conditions do you manage?
I specialize in Intensive Behavioral Therapy (IBT) and Diabetes Self-Management Education (DSME). My curriculum is mapped to the following clinical indications:

- EHR Integration: I can provide these notes via a secure provider portal, fax, or direct upload, ensuring that your patient’s chart reflects their behavioral progress and metrics (A1c, Weight, BP).

How are your services billed?
I offer three flexible pathways to ensure there are no financial barriers to care:
- Direct Patient Pay: I offer a Self-Pay Discount to make education accessible for those without comprehensive coverage.
- HSA/FSA Eligible: My services are a qualified medical expense for patients with Health Savings or Flexible Spending Accounts.
- Practice Integration (1099): I can be contracted as a 1099 employee of your practice. Under this model, your clinic may bill for my services “Incident-To” using appropriate CPT codes (e.g., 98960–98962), creating a new revenue stream for your practice while I handle the patient load.
How do I refer a patient?
Simply provide the patient with my referral card or fax a standard referral form to [Your Fax Number]. I will handle the intake and provide you with a notification once the first session is complete.
